Speech Therapist

Responsibilities:

• Conduct accurate and holistic speech and language specific assessment for clients during admission, periodic care plan reviews and discharge.
• Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making.
• Formulate person-centered and SMART goals in collaboration with clients and relevant stakeholders.
• Design plans and conduct speech and language interventions for clients in a person-centered manner that is also evidence-based.
• Document assessment results, therapy plans, interventions, goals, and outcomes measures according to guidelines.
• Communicate assessment results, intervention plans, goals, outcomes, and discharge plans to clients and relevant stakeholders.
• Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders.
• Facilitate community integration through collaboration with other community partners as required.
• Prescribe assistive devices and equipment according to clients’ needs.

Requirements:

• Minimum Degree in Speech & Language Therapy and must be AHPC registered.
• Minimum 2 years of relevant experience.
• Good team player and able to work independently.
