Overall Job Responsibility
The Service Coordination and Advice team seeks to provide dedicated help to persons with disabilities and caregivers who require disability support and services, especially persons with disabilities who are not enrolled in any government funded disability services.
We hope to enhance clients’ service journey through person-centric support. We assess the needs of the families, coordinate case planning, provide information and advice as well as facilitate applications to suitable schemes or services in SG Enable or other agencies.
In this role, you will have the opportunity to improve service delivery to persons with disabilities and their families. You will lead in stakeholder engagements and/or training of frontline professionals to raise disability awareness, share resources and foster closer collaboration with SG Enable. You will work with the families to navigate suitable post-18 resources, help to continuously review and improve operations. Your good communication skills and ability to foster strong partnerships with relevant professionals will be instrumental to achieve our aim to better support clientsDuties and Responsibilities
1. Case Coordination and Management
- Perform needs assessment and coordinate care planning for persons with disabilities as well as their caregivers by working collaboratively with other professionals to mobilize community resources to support and address the identified needs of persons with disabilities and their families/caregivers. Conduct home visits when necessary.
- Facilitate timely and appropriate service referral and provision to persons with disabilities as well as their caregivers.
- Participates in multi-disciplinary discussion with different stakeholders as part of case coordination and management.
- Attend to and prepare replies to members of public on queries, feedback and appeals as well as provide relevant information and referral services, appropriate schemes that will address the needs of persons with disabilities and their families/caregivers.
- Review and conduct closure of cases upon achieving sustainable service outcomes for clients and the families.
- Maintain proper and up to date case notes and documentation.
2. Stakeholder Engagement
- Plan and implement stakeholder engagement plans including to participate in networking sessions, reaching out to agencies/partners. The purpose is to collaborate and raise awareness on disability support and referral services.
- Contribute towards improving ground practices to support persons with disabilities, such as coordinated care planning or case management. This includes, but not limited to, standardising screening tools and referral workflows, facilitating and conducting Community of Practices by SG Enable.
- Propose and incorporate the relevant competencies and practices into relevant training framework(s) as required.
3. Continuous Review of Work processes
- Co-lead on continual service review process and enhancement.
- Support the review of the ongoing SOPs and operational guidelines for Information and Referral services as well as other business processes.
- Works with Reporting Officer or relevant parties in SGE to identify issues/service gaps, propose recommendations to address the identified issues.
4. Any other duties as assigned
Qualifications, Knowledge and Experience
- Bachelor’s degree in any discipline. Relevant qualifications in social work and allied health profession, psychology will be an advantage.
- Minimal 5 years of work experience with 1 year of supervisory experience in social in social service sector and preferably case management work involving persons with disabilities.
- Must have passion to serve people with multiple disabilities and understand different types of disabilities and the implications on functions across life stages, and the typical experiences faced in navigating the disability landscape.
- Committed, self-driven and dynamic individual who believes in continual learning in diverse skillsets and perseverance to push through projects and plans.
- Strong team player who is collaborative and strives to achieve win-win for the team and stakeholders.
- Strong problem-solving and interpersonal skills.
- Possess analytical, project management and time management skills to build consensus with multiple stakeholders.
- Keen interest in training as well as professional development.
- Proficient in MS Office and preferably Microsoft CRM.
