Social Work Associate
We are looking for a compassionate and dedicated Social Work Associate to join our team in supporting the holistic and person-centered care of our nursing home residents. Working under the guidance of a licensed social worker, you will play a vital role in coordinating psychosocial services, assisting with financial aid processes, communicating with families, and ensuring the emotional and social well-being of our residents.
Key Responsibilities:
- Support resident admissions, orientations, and discharges
- Build rapport with residents and provide emotional and psychosocial support
- Act as a liaison for families regarding non-medical concerns
- Assist in financial aid applications (e.g., NMTS, Medifund, MFEC) and subsidy renewals
- Maintain case files and ensure documentation meets compliance standards
- Assist with end-of-life care planning and bereavement support
- Collaborate with the multidisciplinary care team to ensure integrated service delivery

Qualifications and Requirements:
- Diploma in Social Work, Psychology, Human Services, or related discipline.
- At least 1 year of experience in healthcare or eldercare settings preferred.
- Strong interpersonal and communication skills in English and [other relevant languages].
- Empathetic, patient, and able to work with elderly clients, including those with dementia or complex needs.
- Basic knowledge of local financial assistance schemes and community support services. Proficient in Microsoft Office and case documentation systems
Salary range: $3,000 - $3,500
