Senior / Care Executive (Social Work)

We are seeking a compassionate and dedicated individual to join our team as a Senior / Care Executive (Social Work). In this role, you will be instrumental in supporting the holistic wellbeing of our elderly residents, ensuring their care, dignity, and quality of life. Key Responsibilities

  • Conduct counselling for new referrals, assessing their financial, health, and social circumstances.
  • Coordinate and Support resident admissions, orientations, and discharges, facilitating smooth transitions.
  • Manage the onboarding process, including appointment scheduling, subsidy applications, and addressing general enquiries.
  • Perform frontline counter duties on a rotational basis, handling walk-in and phone enquiries.
  • Build rapport and provide emotional and psychosocial support to residents.
  • Assist in the development and implementation of individualised care plans.
  • Monitor and renew National Means Testing (NMTS) for elderly residents.
  • Serve as a liaison with families, government agencies, and community partners to coordinate care and services.
  • Maintain and submit accurate documentation and reports to relevant authorities.
  • Contribute to the planning, execution, and evaluation of resident-focused programmes and engagement projects.
  • Respond effectively to ad hoc crisis situations, offering timely support and intervention.
  • Provide administrative, operational, and logistical support for client services, outreach, group work, and programme activities.
  • Available to work two weekends per month and on major public holidays.

Requirements

  • Minimum Diploma in Social Work, Healthcare, or a related discipline (Other diplomas may also be considered based on relevant experience or aptitude)
  • 1–2 years of experience in the Social Service Sector preferred. (Fresh graduates with a passion for the sector are welcome to apply)

  • Strong people-oriented approach with excellent interpersonal skills.
  • Good oral and written communication skills (ability to speak Mandarin and dialects is an advantage).
  • Ability to multi-task and remain effective in a fast-paced, dynamic environment.
  • Strong analytical and problem-solving skills to assess situations and make sound decisions.
  • A committed, can-do attitude with a sense of responsibility and initiative.
  • Proficient in report writing and documentation.
  • A reliable and supportive team player
