Job Description
The Family Physician is responsible for the care of clients seen at the Family Medicine Clinic (FMC) Residential Care, and Home Medical services under NTUC Health. He/She will be working in a multidisciplinary environment consisting of care professionals such as care support staff, nurses and allied health professionals. Clinical duties will cut across all 3 services on a regular scheduled and rostered basis.

Professional Responsibilities
- Practice at all times in accordance with the SMC Physician's pledge and abide by the SMC Ethical Code of Conduct
- Keep abreast of medical information and developments through CME events
- Keep abreast of the group's medical practice
Responsibilities at Family Medicine Clinic
- Conduct reviews for chronic medical conditions and practice in a manner aligned with the patient's goals of care, according to best medical evidence and practices.
- Diagnose and treat acute medical conditions, prescribe medications, perform minor surgical procedures and initiate referrals for specialist reviews where necessary
- Work closely with fellow doctors and the clinic Senior Nurse Manager to improve clinical and process quality in matters involving but not limited to Healthier SG and right siting initiatives.
Responsibilities at Nursing Homes
- Conduct medical rounds to review nursing home residents on a regular scheduled basis to attend to their chronic medical needs and acute conditions in a manner aligned with the patient's goals of care, according to best medical evidence and practices
- Write medical reports, memos, referrals and administrative forms where required for purposes of application to schemes beneficial to residents and/or to correspond with other healthcare and non healthcare services
- Practice with a patient centered approach, and engage with the resident or next of kin where necessary in order to develop a care plan aligned with the residents' values and desired outcomes
- Participate in local clinical quality improvement and clinical efficiency projects together with fellow doctors and nurses
- Participate in engagements and collaborations with the public health institutions to improve the care of nursing home residents
Responsibilities at Home Care
- Deliver patient centered medical care in a manner consistent with the patient's individual care goals
- Commute to patients homes and deliver care in a manner consistent with the patient's individual values and care goals
- Perform comprehensive medical assessments and develop care plans which will direct the care delivered by a multidisciplinary team of doctors, nurses, allied health professionals and social workers
- Identify patients approaching end of life, initiate EOL conversations where appropriate and provide early generalist palliative care
Qualifications
- Basic medical degree recognized by the Singapore Medical Council
- Full registration with the Singapore Medical Council
- Graduate Diplomas in Family Medicine / Palliative Medicine / Geriatric Medicine or MMed (Family Medicine) would be an advantage
- Possess good communication and interpersonal skills
- Willingness to serve and connect with the elderly and underprivileged
- Interest in medical innovation and novel care delivery models
- Relevant Graduate diplomas in Geriatric Medicine, Palliative Medicine or Mental Health
- Experience working with elderly or palliative patients
- Experience with medical innovation and/or novel care delivery models
- Proficiency in Google Workspace tools would be advantageous
