DESIGNATION : ICCP Assistant Programme Manager
RESPONSIBILITIES
The ICCP (Integrated Community Care Partner) Assistant Programme Manager is responsible to ensure the effective implementation of strategies to meet programme objectives and service models. The ICCP Programme Manager will evaluate programme performance, ensure adherence to the funding requirements, monitor programme expenses, prepare and submit progress reports, conduct quality assurance activities, and develop strategies to collaborate with Community Care Providers within the sub-region.
Programme Management
- Initiate and plan programmes with the Community Care Providers within the sub-region to create wider variety of programmes for the seniors
- Collaborate with Community Care Providers within the sub-region to support seniors through use of standardised care assessment tool that facilitate a co-development of a shared care plan
- Determine allocation and optimization of resources for seamless, efficient and effective delivery of coordinated care for the seniors
- Establish a framework for Community Care Providers to share information on their services with one another
- Manage new referrals while existing clients continue to receive care from their current Providers
- Develop workflows with the Community Care Providers to facilitate a seamless care approach to support the seniors
- Establish joint accountability to meet KPIs and service level agreements set by funders
- Set goals together with Community Care Providers on improving the health and well-being of seniors in the sub-region
- Establish clear data sharing mechanisms to define how data will be collected, used and shared among Community Care Providers, Public Healthcare Institutions, other healthcare Providers and other organisations
- Adapt and incorporate best practices and trends in programmes
- Review and recommend improvements to funders, organisational policies and processes that impact the programme
Stakeholder Management
- Foster a collaborative partnership where partners align and agree on goals and outcomes that will best support seniors of the sub-region
- Establish key partnership with MOH, AIC, hospitals, primary care doctors to coordinate our support for Seniors
- Build trust, openness and transparency among partners
- Manage communications and feedback from stakeholders and team members
Compliance
- Incorporate processes for the collection, use and disclosure of personal date, to ensure compliance with Personal Data Protection Act (PDPA)
- Ensure that consent clauses, statements, declarations and notifications are sufficiently robust for usage and onward sharing to third parties
- Review and update the status of KPIs to management regularly
- Monitor programme expenditures to ensure it is within the budget.
- Evaluate KPIs to ensure KPIs met within the timeline as per funding requirements
- Vet/Prepare reports and ensure information/data is accurate
QUALIFICATIONS
- Possess Degree from an accredited institution
OTHER INFORMATION
Relevant Experience
- At least 5 years of experience in social service / programme management / healthcare environment or related fields.
- Candidates with less years of experiences may be considered as well
Competencies
- People-oriented, good interpersonal and relation building skills
- Excellent communication and written skills for report and proposal writing
- Analytical ability & problem-solving skills to analyse information quickly and accurately, formulate decisions and make recommendations
