Job Description

As a member of the Care Navigation team, you will contribute to customer service excellence by demonstrating empathy towards seniors and their family members who contact NTUC Health, understanding their circumstances, and recommending appropriate services to meet their needs. Training will be provided to equip you with the necessary knowledge to perform your role.

Responsibilities:

  • Demonstrate a strong understanding of eldercare services that meet the needs of seniors.
  • Attend to customers' calls and email inquiries.
  • Assess customers' needs, recommend suitable services, and facilitate their enrolment.
  • Conduct needs assessments, including observations and interviews with seniors and their families, as required.
  • Collaborate closely with service providers to ensure timely enrolment and ongoing support for seniors' changing needs.
  • Participate in meetings, take minutes, and track follow-up actions.
  • Prepare reports for submission to regulators and other stakeholders.
  • Represent NTUC Health at occasional roadshows to promote eldercare services.

Qualifications

  • Have strong communication skills.
  • Are meticulous and detail-oriented.
  • Have at least 2 - 3 years of relevant experience.
  • Hold a diploma or degree in any discipline.
  • Have prior experience in healthcare or community care (an advantage).
