Social Worker – CREST (West Region)

CREST focuses on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary.

The Social Worker works in the field of Social Work to provide interventions and programmes, perform community development work and uplift the professional practice in Social Work. He/She conducts casework and group work as well as implement and evaluate community development activities and programmes to address service gaps in the community. He/She also provides support and guidance to Care Associates in the team and works in collaboration with other agencies and ministries in the course of his work.

Main Duties & Responsibilities:

Conduct Casework and Counselling

• Conduct needs and risk assessments for complex cases
• Provide case management, counselling, psychosocial interventions and psychoeducation for clients with mental health concerns and/or dementia, and their families/caregivers
• Formulate and implement casework and counselling intervention plans
• Evaluate progress and effectiveness of casework and counselling
• Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities

Perform stakeholder engagement and partnerships

• Build professional relationships with clients, significant others, and other professionals
• Collaborate with other agencies to pool resources for clients

Conduct Groupwork

• Review needs to plan and conduct group work to address issues of target groups
• Plan, execute and evaluate mental wellness programmes and group work intervention

Perform Community Development Work

• Scan the environment and community need to develop community development initiatives
• Mobilise community resources, develop and implement community development initiatives

Support and Guidance

• Provides support and guidance to Care Associates in the team
• Discussions with Programme Executive on analysis, planning and implementation of programmes and services to meet clients’ needs

Others

• Support the operations of Viriya Mental Wellness Hub located at Kent Road
• Planning and executing events within the Division and organisation
• Any other job tasks may be assigned, if needed
