Working Hours: Mon to Thurs (8am to 6pm) / (8:30am - 6:30pm), Fri (8am to 5pm) / (8:30am - 5:30pm)
Location: Bukit Batok

Job Responsibilities

• Develop individualise care plans for residents
• Execute the care plan and monitor progress
• Coordinate care for residents with relevant stakeholder

Requirements

• Degree in Social Work
• Preferably experience in Social Service Sector or related field
