Key Responsibilities and Duties
- Comply with approved standards of nursing practice and organisational policies.
- Assist the Registered Nurse (RN) in supervising care staff and healthcare attendants.
- Provide basic nursing care to residents.
- Observe, monitor, and report changes in residents’ conditions and care needs.
- Ensure delivery of care and nursing interventions in accordance with individualised care plans.
- Conduct nursing rounds jointly with the RN.
- Supervise care staff to ensure residents’ comfort, personal hygiene, and dignity.
- Supervise care staff in checking, monitoring, and recording residents’ vital signs accurately.
- Maintain accurate and timely documentation of care rendered.
- Assist the RN in medication administration and ensure compliance with safe medication practices.
- Guide and assist care staff in the safe transfer, positioning, and mobility of non‑ambulant residents.
- Supervise care staff in diet serving and feeding for residents requiring assistance.
- Assist in the admission, assessment, and orientation of new residents.
- Perform basic rehabilitative activities, including range‑of‑motion exercises and assisted ambulation.
- Arrange and escort residents for prescribed therapies (e.g. occupational therapy).
- Coordinate and arrange residents’ medical appointments and transportation.
- Accompany residents to hospitals or polyclinics for follow‑up appointments when required.
- Initiate appropriate actions during emergency situations and promptly escalate to the RN.
- Work collaboratively with the RN to maintain a safe, clean, and therapeutic environment within the home.
Secondary Responsibilities and Duties
- Participate in clinical service improvement projects.
- Participate actively in infection prevention and control initiatives.
- Support training, guidance, and development of care staff.
- Perform any other duties and responsibilities as assigned by the Senior / Nurse Clinician, Director of Nursing (DON), or Chief Executive Officer (CEO).
Requirements
- Locally Trained National ITE Certificate in Nursing.
- Foreign Trained Diploma/Degree in Nursing
- EN enrolment with Singapore Nursing Board.
