Locations available: West side polyclinics OR Queensway

Job Responsibilities:

Attend to telephone and email enquiries for Medical Social Service.
Provide administrative support to the Medical Social Worker on referrals, accompany social worker on field visits and assist in interpretation if required.
Gather information and conduct preliminary investigation on patients' needs to facilitate intervention by Medical Social Worker.
Process application and ensure submission are in compliance.
Maintaining the work statistics and compile the data for submission to Ministry of Health.
Handle external correspondences such as MP letters and CDC letters.

Requirements:

Relevant education qualification in Social Work or equivalent
At least 1 year of working experience in community health
Willing to travel between polyclinics occasionally and do ad-hoc home visitations together with Medical Social Worker
