Healthcare Care Coordinator

Responsibilities
- Build and maintain positive relationships with patients, caregivers, and healthcare team members.
- Support patient care coordination and follow-up activities.
- Use IT systems to track, document, and monitor patient care and clinical indicators.
- Conduct basic health screenings and measurements.
- Promote preventive health screenings and vaccinations in line with national health initiatives.
- Provide basic health education on healthy lifestyles, disease prevention, and self-monitoring of health conditions.
- Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucose meters, and weighing scales.
- Assist patients in accessing healthcare and community resources to improve their overall well-being.
- Leverage technology to enhance patient engagement, health literacy, and access to care.
- Other ad hoc duties assigned by the supervisor
Requirements
- NITEC, Diploma, O-Level, or A-Level qualification.
- Good verbal and written communication skills.
- Pleasant personality with strong interpersonal skills.
- Positive attitude, self-motivated, and a good team player.
- Interested in patient care and community health.
- Proficient in Microsoft Office applications (Word, Excel, and PowerPoint).
- Comfortable using IT systems and digital tools.
