Job Description

The Social Worker/Case Worker is a trained Social Worker. He/She is qualified and trained to assist in providing interventionsand programmes, performing community development work and community professional practice in social work. The requires the candidate to support in coordinating casework, group work and community development activities and programme, supporting community events and performing administrative functions

A responsive, adaptable and astute staff who is able to work with a collborative team from internal colleagues and external agencies, ministries in the course of his work.

Roles & Responsibilities

1.Case and Care Planning

  • Attend to all Intake Assessments for DSA.
  • Linking resources and services internally and externally according to the case needs.
  • Casework intervention includes collaborative practice across discipline and sectors.
  • Initiate care planning between different services in DSA and monitor and evaluate the progress of case

2. Case Management

  • To be able to attend to crisis and risk of cases in a timely manner.
  • To be able to report to the respective authority accordingly where the needs arise.
  • Able to submit social reports to support families who may need support financially or emotionally.
  • To be able to furnish and submit case session in a timely manner.

3. Community Outreach & Group Work

  • Carry out and implement needs assessment activities in the community or for the DS specific programme.
  • To evaluate feedback and review for participants, community partners and collaborators to help DSA understand trends, challenges and gaps
  • Work closely with internal and external stakeholders, service providers, relevant government agencies, academic institutions (schools / Institutions of Higher Learning), community & social service agencies, for the purpose of networking, resource sharing, referral, collaborations, and support assistance.

4. Learning and Development

  • Participate and contribute to relevant workshops, conferences and seminars pertaining to the development of persons with Down Syndrome.

5. Reports and Updates

  • Prepare and collate monthly reports within SAS, initiatives and any other relevant training information as designated or required by the Management or the Board of Management.

6. Meetings and Secretarial Support

  • Attend relevant meetings and to provide secretariat support as necessary.
  • Carry out and implement any other tasks / duties assigned by the Manager or the Executive Director.

Requirements

Degree in Social Work / Undergoing Social Work Course

Case Management, Group work and Research

2 year of relevant experience. Experience in running / managing cases, groupwork and programmes or a similar capacity in the social service sector will be advantageous.
