Staff Nurse (Home Care)

About the Role
We are looking for a compassionate and proactive Home Care Nurse to provide nursing services to patients in their homes while supporting daily operational and administrative functions. This is a mobile role that combines clinical nursing, patient coordination, customer service, and administrative responsibilities.
The ideal candidate is comfortable working independently in the community, enjoys interacting with patients and families, and is willing to support the smooth running of home care operations.
Key Responsibilities
Clinical Nursing Care
Conduct patient assessments and monitor health conditions during home visits.
Administer medications, injections, wound care, and other prescribed nursing procedures.
Perform basic health screenings and vital signs monitoring.
Educate patients and caregivers on care plans, medications, and disease management.
Maintain accurate and timely clinical documentation.
Escalate clinical concerns and changes in patient condition to the appropriate healthcare professionals.
Patient Registration & Coordination
Register new patients and collect required documentation.
Assist with appointment scheduling and care coordination.
Liaise with patients, family members, caregivers, and healthcare providers.
Follow up on patient enquiries and service requests.
Administrative & Operational Support
Process service forms, consent forms, and nursing records.
Prepare and submit visit reports and documentation.
Assist with inventory tracking and medical supplies management.
Support billing administration and reconciliation processes.
Collect payments from patients when required and ensure proper documentation and accountability.
Maintain confidentiality of patient and company information.
Mobile Community-Based Work
Travel to patient homes and care facilities across Singapore.
Manage daily visit schedules efficiently.
Provide professional representation of the company during patient interactions.
Job Requirements
Must be registered with Singapore Nursing Board
Good interpersonal skills with a pleasant personality
Able to work in a dynamic environment as well as being flexible and open to changes
Preferably experience with assisting different related procedures
Working Hours: 5.5-day work week. Weekend and public holiday duties may be required based on operational needs and patient schedules.
Work Arrangement: Mobile role involving travel to patients' homes and community locations.
