- Coordinate patient care and follow-up appointments.

- Liaise with doctors, nurses, caregivers, and community partners.
- Assist patients in using digital health tools and home monitoring devices.
- Maintain accurate patient records and documentation.
Requirements
- Diploma in a healthcare-related field or relevant work experience.
- Strong communication and interpersonal skills.
