Senior/ Staff Nurse (Home Care)

Responsibilities

• Deliver safe and individualized nursing care in accordance with professional standards and regulatory requirements.

• Oversee and mentor new and junior staff to enhance the knowledge and effectiveness of the nursing team.

• Manage complex patient care through comprehensive assessments, implementing care plans, and coordinating with healthcare providers.

• Build and maintain rapport with patients, families, and caregivers, ensuring effective communication throughout the care process.

• Assess the home care requirements and facilitate the provision of necessary equipment and resources.

• Perform essential nursing tasks, including medication administration and wound care, while ensuring comprehensive documentation.

• Participate in Multidisciplinary Team meetings and collaborate with various teams to ensure seamless patient care delivery.

• Keep abreast of the Singapore Nursing Board's Standards of Practice and uphold high ethical standards in all interactions.

• Maintain safety protocols and efficient use of medical supplies and equipment while providing exceptional customer service.

Requirements

• Diploma in Nursing from a recognized local Polytechnic; registration with the relevant nursing authority is mandatory.

• Preferably possess post-registration experience in a hospital or hospice environment.

• Familiarity with IT systems and a commitment to continuous professional development.

• Strong interpersonal and communication skills with a keen attention to detail.

• Effective time management abilities and capacity to thrive under pressure as a collaborative team member.
