Overall Job Responsibility
In this role, you will develop holistic care plans through comprehensive bio-psycho-social assessments of families through the person and family-centric approach. In this role, you will provide case coordination to safeguard the long-term welfare and financial security of persons with disabilities, including facilitating future care planning (FCP), setting up of SNTC Trust where relevant and conducting regular reviews of care plans for SNTC Trust beneficiaries.
Duties and Responsibilities
- Engage families in FCP conversations and help them plan for persons with disabilities' long term care needs.
- Assess eligibility of clients for sponsorship schemes and facilitate applications.
- Assess eligibility and suitability of the Special Needs Trust for the client and facilitate Trust setup accordingly.
- Annual Trust review, formulation and implementation of plans to earmark assets to the Trust.
- Timely evaluation and review of financial care plans and work with clients to modify the care plans.
- Navigate through various help systems and facilitate the access to necessary resources and services for clients.
- Service all SNTC clients assigned from time to time including making home visits to SNTC trust beneficiaries.
- Provide relevant documentation and prepare all applicable reports in a timely manner.
- Answer and attend to hotline and/or walk-in queries.
- Prepare and provide support for outreach talks.
- Mentor sector professionals in social service agencies to support clients in FCP. This could include deployment to social service agencies and working alongside existing case manager/ social workers to support families in FCP.
- Conduct workshops and training to support clients in putting future care planning instruments in place.
- Any other duties as assigned.
- Relevant qualifications in social work will be an advantage
- At least 3 years of work experience in social service sector
- Good analytical and problem-solving skills
- Good verbal and written communication and interpersonal skills with the ability to engage clients
- Good organization skills, meticulous and diligent in managing competing demands
- Team-player with a growth mindset and displays initiative
