Community Support: CCMS (Community Case Management Services) | CREST (Community Resource, Engagement and Support Team) | Henderson Home
The Nursing Home is committed to a Person-Centred Care model, aiming to transform care environments to promote genuine human connection and resident empowerment for our seniors. The Medical Social Workers are integral to this model, providing psychosocial and financial assessments, casework, and interventions. They also collaborate with the multi-disciplinary team of stakeholders to review individual residents' medical, nursing, therapy, and social aspects as part of delivering person-centric care. This support is crucial throughout the residents' journey, from their admission to their entire stay in the Home.
Admissions
- Screen referrals from AIC and work with the nursing teams and relevant stakeholders to assess suitability of the referral.
- Co-ordinate and complete onsite referral assessment for any care concerns, if necessary.
- Provide supervision and support to the case management executive(s) and care concierge to complete the pre-admission processes.
Case work and clinical interventions
- Work with the Multi-disciplinary team and next of kin to ease the resident's transition and adaptation to the new living environment.
- Provide bio-psychosocial spiritual (BPSS) assessment and evaluate resident's financial status to develop comprehensive care plans with the multi-disciplinary team.
- Provide psychosocial interventions and counselling support to the resident and/or their next of kin, and facilitate communication and visits to preserve family bonds.
- Facilitate family conference and conversation for discussion on care planning eg discharge, advance care planning, lasting power of attorney, deputyship; optimising family involvement in residents' care journey.
- Provide caregiver support resources to support families where required.
- Review supporting documents for referral to other care options and collaborate with multi-disciplinary team for assistive equipment as recommended by the therapist where required as part of discharge planning.
Financial Assistance Schemes
- Work with resident or next-of-kin on assessing eligibility for subsidies and financial considerations
- Review cases and facilitate meetings for cases requiring financial assistance (e.g. deviation, Medifund, MFEC - complex cases)
- Assist case management executive in applying for Means Test, AIC Long Term Care Schemes, MFEC - straightforward cases)
Reports and administrative duties
- Work with case management executive to ensure residents' profile and movement are updated in database(s)
- Review next-of-kin details and update residents' social report
- Collaborate with Finance to review residents accounts receivable status when required
- Manage and supervise team members in case management team
Qualifications
You should:
- Have a degree or Post Graduate Diploma in Social Work
- Be registered with Singapore Association of Social Workers (SASW)
- Social work in healthcare background preferred
- Have at least 3 years of relevant experience in intermediate and long term care settings
- Good interpersonal and problem-solving skills
- Caring, positive, compassionate and self-disciplined individual
- Ability to work well in a team
- Proficient in Google Workspace (i.e. Gmail, Google Docs, Sheets, Slide)
