Job Responsibilities
- Patient Care Management: Taking comprehensive histories, performing physical examinations, executing clinical procedures, and completing follow-up reviews.
- Patient & Caregiver Education: Communicating treatment plans, conducting discharge teaching, and training families on care.
- Clinical Supervision: Guiding Junior Enrolled Nurses, Nursing Aides, and Care Associates.
- Infection Control & Documentation: Maintaining clinical records and upholding standard precautions during home visits
Requirements:
- Valid SNB and BCLS
- Able to commit at minimally 3-5 days a week, 8:30am-6pm
- Home care/nursing home experience required OR interRAI certified
