Assistant Care Coordinator

Responsibilities

  • Facilitate patient selection, counselling, enrolment and referrals to primary care partners.
  • Provide training and develop workflows to assist hospital departments in right-siting efforts.
  • Support continuous patient care within the community.
  • Monitor and manage the transition of patients from polyclinics to general practitioner partners.
  • Input and maintain accurate data in the tracking database.
  • Engage patients in health promotion activities and national initiatives as needed.
  • Execute additional tasks as assigned by supervisors and the Head of Department (HOD).

Requirements

  • Possess a Diploma in a health or social-related discipline.
  • Demonstrate the ability to multi-task and work autonomously as well as collaboratively.
  • Exhibit strong organisational, coordination, interpersonal, and communication skills.
  • Respond effectively to inquiries or complaints from doctors, staff, and patients.
  • Proficient in Microsoft Office applications.
