Job Responsibilities
- Assess needs of seniors with care/service needs and formulate care plans for seniors with care/service needs.
- Assist in the development and implementation of programmes for seniors through data collection and the conduct of basic analysis.
- Coordinate care for the seniors with relevant stakeholders.
- Documentation of case notes.
- Facilitate timely and appropriate service referral and provision to seniors.
- Participate in case discussions with external service providers.
- Provide case management services.
- Perform needs assessment of seniors living in the community.
- Prepare data for submission to relevant authorities.
- Degree in Psychology or Social Work
- With relevant working experience is an advantage
