Medical Social Worker

Location: Bright Hill

Working Days: Monday - Friday

Working Hours: 8.30am - 6pm

Salary: $5,000 – $6,500

Key Responsibilities

1. Intake, Assessment and Admission Support

  • Partner with Nursing staff to conduct intake screening for prospective residents/clients.
  • Perform psychosocial assessments covering residents' and families' social, emotional, financial, caregiving and support needs.
  • Identify residents and families requiring ongoing psychosocial support.
  • Participate in pre-admission and admission assessments.
  • Support newly admitted residents and families during admission.
  • Conduct InterRAI assessments and related evaluations.
  • Document assessments, interventions and care plans in the designated systems.

2. Casework, Counselling and Care Management

  • Provide counselling and psychosocial interventions to residents and families.
  • Conduct case management and coordinate referrals to community resources, healthcare providers and government agencies.
  • Provide information on government schemes, financial assistance and community support services.
  • Follow up on financial assistance applications where required.
  • Manage assigned caseloads and maintain case documentation.

3. Advance Care Planning (ACP)

  • Facilitate and document Advance Care Planning (ACP) discussions.
  • Conduct ACP facilitation sessions as required.
  • Support ACP implementation for long-term residents.
  • Provide information on Lasting Power of Attorney (LPA) where applicable.

4. Discharge Planning and Care Transition

  • Develop and implement discharge and transition plans with the multidisciplinary team.
  • Coordinate referrals to nursing homes, community hospitals, hospices, home care services and relevant agencies.
  • Support discharge planning and long-term placement arrangements.

5. Respite Care Coordination

  • Assess respite care referrals.
  • Coordinate admissions, assessments and care planning.
  • Support placement planning and discharge arrangements.

6. Crisis Intervention

  • Conduct risk assessments and interventions for: Suicide risk / Family violence / Caregiver burnout / Grief-related situations
  • Develop intervention plans and provide family mediation where required.

7. Palliative Care and Bereavement Support

  • Facilitate End-of-Life (EOL) discussions.
  • Document end-of-life care preferences.
  • Coordinate funeral arrangements were required.
  • Provide bereavement support and referrals.

8. Resident Engagement

  • Facilitate group programme and support groups.
  • Organize resident committee meetings.
  • Support resident engagement activities and community programmes.

9. Multidisciplinary Collaboration

  • Participate in multidisciplinary meetings and case conferences.
  • Collaborate with healthcare institutions, community agencies and service providers.

10. Administration

  • Maintain records, reports and documentation.
  • Support quality improvement initiatives and departmental projects.
  • Participate in professional development activities.

11. Other Duties

  • Attend to enquiries and service referrals.
  • Perform other duties assigned by Management.
  • Bachelor's degree in social work / BA (Major in Social Work) / Graduate Diploma in Social Work.
  • Registered Social Worker with the Social Work Accreditation and Advisory Board (SWAAB).
  • 3–5 years of relevant experience, preferably in a hospital or community hospital setting.
  • Experience working with elderly and people with dementia.
  • Certification as an Advance Care Plan Facilitator is an advantage.
