Physiotherapist [WCAN]

Conduct initial physiotherapy assessments using appropriate assessment tools and outcome measures.

Develop individualised care plans and establish treatment goals based on patients' needs.

Provide physiotherapy interventions in patients' home settings.

Educate caregivers on patient care techniques and prescribe appropriate assistive equipment.

Coordinate discharge planning and referrals to relevant services where necessary.

Participate in monthly home therapy meetings and contribute to case discussions.

Conduct therapy-related in-service training and attend departmental training sessions.

Perform other administrative and clinical duties as assigned.
